The central midfielder has signed a deal until 2024 with 'Die Werkself'.
"Bayer 04 have always played attractive attacking football. This style of play suits me and I'm really looking forward to being a part of this team," Demirbay said, as quoted by Bayer Leverkusen.
Bayer Leverkusen sporting director Simon Rolfes said:
"In Kerem Demirbay, we have secured a technically savvy and strategically active midfielder who's also dangerous in front of goal. Kerem has been one of the best performing players at Hoffenheim and will also be an important factor for us to meet our requirements in the coming season".
The transfer fee is undisclosed, but has been reported to be €28 million.
Demirbay has scored four goals and made 10 assists in 30 appearances for Hoffenheim this season.
