Achraf Hakimi missed a glorious chance in the first half after he was played through on goal, sending a shot straight at Fortuna goalkeeper Florian Kastenmeier.
'Die Schwarzgelben' thought they had scored midway through the second half, but Raphael Guerreiro's goal was controversially ruled out for a handball after a VAR review.
Towards the end of the game, Steven Skrzybski nearly put the hosts ahead, but his shot hit the post.
Instead, it was Dortmund who ended up taking all three points when second-half substitute Erling Haaland headed the ball into the back of the net in injury time.
The result means that BVB are four points behind league leaders Bayern München before the Bavarians take on Borussia Mönchengladbach later on Saturday.
Elsewhere, Werder Bremen took an important win in their bid to avoid the drop as they crushed relegation rivals Paderborn 5-1. Wolfsburg and Freiburg drew 2-2, FC Köln lost 1-2 at home to Union Berlin while Hertha BSC were beaten 1-4 at home by Eintracht Frankfurt.
