The German tabloid reports that the 26-year-old central defender will undergo a medical today ahead of the move.
Koch will join Eintracht on an initial loan deal, with a permanent transfer to be considered at the end of the season, when his contract with Leeds expires.
Bayer Leverkusen have also been linked with Koch, who previously played for Freiburg.
