Eintracht Frankfurt's successful team from last season seems to be slowly falling apart. Their coach, Niko Kovac, has joined Bayern München while Lukas Hradecky (Leverkusen), Marius Wolf (Dortmund) and Omar Mascarell (Schalke 04) have all left the club. Now, Kevin-Prince Boateng and Ante Rebic could also leave.

According to Bild, Boateng is nearing a move to Italian Serie A side Sassuolo. The 31-year-old wants to spend more time with his wife and son who lives in Milano - a two and a half hours' drive from Sassuolo.

Boateng has spent just one season at Eintracht Frankfurt after joining the club from Las Palmas last summer.

Meanwhile, Frankfurt have received several offers for Ante Rebic, who is currently playing for Croatia at the World Cup, Sport1 understands.

The 24-year-old has been linked with Bayern München as well as Tottenham, Everton and Huddersfield Town from the English Premier League. Now, he has reportedly also drawn interest from Italy.

Rebic's contract runs out in 2021, and Frankfurt are hoping for a transfer fee of around €30 million.
